A thick, yellowish nail with splotchy white areas is a classic presentation for a fungal toenail. In the early stages the toenail is only slightly discolored, or slightly thickened. In the late stages the nail can become very thick and has a tendency to “grow up” more than in grows out. The nail will be very discolored, typically dark yellow-brown and will start to change shape and crumble. In some cases the nail becomes very loose and many will lose the nail, only for it to regrow with the fungus.

How do we treat bipolar disorder? Specifically, how do we treat mania or depression associated with bipolar disorder? The treatment of these two clinical states is not the same.

The treatment of mania is dependent upon its severity and acuity. For mild to moderate mania, mood stabilizers such as lithium and valproic acid (Valproate) are still the standard of treatment and may be sufficient to contain the symptoms. Lithium starts to work after 10 to 14 days while valproic acid, about 7 to 10 days.

While on a recent trip to the grocery store, I happened to hear a mother telling her teenage daughter not to answer her ringing cell phone. Of course, the daughter explained to her mother that “she just had to answer it” As the mother was in the middle of stating how she barely gets two minutes of her daughters time in a day, and her daughter answering the ever so “important” call anyways, all I could think was how much I could relate with this poor woman. In fact, I’m sure most parents with teenage children can relate to her as I did.
